Preliminary investigation finds train crash near Tembisa due to 'human error'

Metrorail says its preliminary investigation into the fatal train crash near Tembisa has revealed that the accident was as a result of "human error".
One person was killed and 243 people injured when two trains collided head on shortly after 13:00 today.
One of the train drivers has also been injured and admitted to hospital.
The deceased is a man in his 20s.

Gauteng Metrorail spokesperson Lillian Mofokeng says: "A full board of inquiry will be set up to ensure we establish the root cause of the accident."
The commuter rail service says it will offer support to those involved the accident.
"We have social workers who will provide assistance to all those involved in the accident."

The railway lines are closed as authorities try and clear the wreckage.
A bus service is in operation between these stations for today, Mofokeng confirms.
She says she is at this stage unable to confirm whether the rail service between these stations would be operational by Tuesday morning.
