Denver train crash caused by human error

The Minister of Transport, Dipuo Peters, briefed the media on Monday on the findings of Denver train collision which left 240 people injured.

In April 2015 a MetroPlus train that had stopped at the Denver Train Station in Johannesburg was rear-ended by a Metro Express train.
One person died while 240 people sustained various injuries.
Peters said there were a number of contributing factors, but that human error played a significant role.
"The findings of the investigations are systemic and include the following: issues of lack of maintenance, ineffective communication, ineffective human factors management, ineffective allocation of capital funding," Peters said.
She added that Prasa would have to undergo a complete overhaul to ensure the safety of commuters.
"Prasa will be investing approximately R172 billion over a period of 10 years which has already commenced in the 2013/2014 financial year," she said.
Prasa said it would first study the report before taking any action.
