A train towing vehicle was towing a train from the tracks to the Gautrain workshop when it derailed and crashed into a building in Midrand.

Paramedics say three male patients were injured in the crash.

"Tragically the driver of the towing vehicle sustained severe injuries and died at the scene. Two occupants of the train that was being towed, a driver and a technician, sustained mild to moderate injuries," says Netcare 911 spokesperson Nick Dollman.

The police say the incident took place in the early hours of Wednesday morning and it is investigating a case of culpable homicide.

"We opened a culpable homicide case to find out whether there is any negligence on the part of any person who we find to be responsible of the cause, not exactly to the deceased person but who contributed to the negligence," says police spokesperson Molefe Mogodi.

The Gautrain's Kesagee Nayager says an investigation is underway.

"A full investigation is underway to determine the root cause of the incident and we will fully cooperate with the Railway Safety Regulator who will conduct its independent investigation," says Nayager.
