He testified in January 2019 that Jabulile Sishuba was one of the officials at the department who received monthly bribes from late Bosasa CEO Gavin Watson.
Sishuba subsequently applied to cross-examine Agrizzi.
"I am truly sorry and as soon as it was brought to my attention that I had confused names I apologised. There was no malice intended, I think it was rough, it was confusing with everything happening and I understand that it is damaging," Agrizzi told the commission on Friday.
He also confirmed he had personally apologised to Sishuba and that she had accepted his mea culpa.
