Police believe the driver of one of the vehicles was traveling at a high speed and lost control before slamming into the vehicle carrying the five occupants.

The crash occurred near the Golden Highway off-ramp.

Johannesburg metro police spokesperson Wayne Minnaar says the driver of the Audi is facing five counts of culpable homicide.

"Two men, a woman, one year old child and an eleven month baby died when a silver Audi collided head on with a Polo travelling on the N12 west," says Minnaar.

"Five occupants of the Audi, including the driver, got seriously injured and had to be taken to Chris Hani Bara hospital."

A further investigation into the crash has been launch by police.
